This template from the United Nations Office on Drugs and Crime (UNODC) is designed to take the user through a step by step process of evaluability assessment. 

"The assessment answers the following questions:

  1. Does the quality of the design of the Programme allow for the evaluation?
  2. Are the results of the programme verifiable based on the planned collection systems?
  3. Would the evaluation be feasible, credible and useful?"
