This report on a community-based survey about Jamaican youth risk and resiliency behaviour conducted in 2006 on behalf of USAID contains an example of a data use calendar as Appendix 9 (page 158).
It lays out in matrix form a number of decisions or issues, along with who the decision-maker and key stakeholders are, what information is required, and what the next steps are, and the timeline for taking these steps.
